When coverage ends

Your coverage under QPAT's Long-term Disability Plan automatically ends at midnight on the earliest of the following dates:

• the termination date of the Plan or benefit;
   
• the date on which you cease to meet the eligibility requirements;
   
• the date on which you cease to participate in QPAT's Health Plan, unless you are exempted from such coverage;
   
• the date on which you are incarcerated after committing a criminal offence for which you were found guilty; and
   
• the date of your death.

Coverage also ends on your 63rd birthday, provided you are not already disabled.
