How do I make a claim?

  1. Complete the Claim Request form to apply for benefits and a waiver of premiums.

  2. Return the form within 90 days immediately following the date of your disability, to:

    Industrial Alliance
    Claims Department
    P.O. Box 790, Station B
    Montreal, Quebec
    H3B 3K6

  3. At the same time or within five months following the date benefits become payable, provide Industrial Alliance with proof that you are totally disabled.

Industrial Alliance reserves the right to require additional proof whenever it deems necessary and to have a physician of its choice examine you.

What is the deadline for submitting claims?

Ninety days immediately following the date of your disability.

Failure to submit a claim promptly may affect your entitlement to benefits. Please make sure the school board gives you the information you need to submit your claim to Industrial Alliance.

What is the Plan's policy number?

The policy number is 97000 or 97001. Please refer to your wallet card for the applicable number.
