Life Insurance Plan

On the earliest of the following dates:

   
upon retirement (you can continue to be insured for an amount of $10,000, provided you pay the required premiums);
   
the date you choose to no longer participate in the Plan;
   
the date you cease to participate in the Health Plan, unless you had opted out;
   
upon your death;
   
the termination date of the Plan; and
   
the date on which you are incarcerated after committing a criminal offence for which you were found guilty.
Additional Life Insurance Plan

On the earliest of the following dates:

   
the first day of the month coinciding with or following your 65th birthday;
   
upon your retirement;
   
the date you choose to no longer participate in the Plan;
   
upon your death;
   
the termination date of the Plan; and
   
the date on which you are incarcerated after committing a criminal offence for which you were found guilty.

When does my dependents' coverage end?

Your dependents' coverage under QPAT's Dependent Life Insurance Plan automatically ends at midnight on the earliest of the following dates:

   
your retirement date (you can continue to insure your spouse for $5,000, provided you pay the required premiums);
   
the termination date of the Plan;
   
the date on which your coverage ends;
   
the date on which your dependents cease to meet the definition of spouse or dependent children; and
   
the first day of the month after your employer has received written notice from you to the effect that you are no longer covering your dependents under QPAT's Dependent Life Insurance Plan.

Can I convert my coverage to an individual policy?

Yes, in the following instances:


If your coverage ends
because …
You can convert your life insurance coverage, provided you are …
Your employment or your group membership has been terminated (not because you simply opted out of the Plans)

Under age 65

Your group coverage is cancelled or your group policy was modified

Under age 65, and you have been covered for at least five consecutive years

If you decide to convert your life insurance coverage, take note of the following important considerations:

   
You have 31 days after your coverage ends to convert all or part of your life insurance coverage in writing.
   
Coverage must be converted to an individual life insurance contract that Industrial Alliance usually issues.
   

You may choose one of the following types of insurance:

     
  permanent;
     
  term to age 65; or
     
  one-year term, convertible into permanent or term to age 65 at the end of one year.
   
The total amount that you can convert from both the Life Insurance Plan and the Additional Life Insurance Plan combined (including any other group insurance policy issued by Industrial Alliance) is equal to the smallest of the following amounts:
     
  the amount selected at the time of conversion;
     
  the amount for which you were insured immediately prior to the termination of your insurance;
     
  the difference between the amount for which you were insured immediately prior to the termination of your insurance, and the amount for which you are eligible under a new group life insurance contract; and
     
  $200,000.
     
The individual insurance policy will not contain a disability clause or an accidental death and dismemberment clause.
     
The premium will be based on Industrial Alliance's standard rates in effect for your age and class of risk.
   
When you submit your written request to Industrial Alliance to convert your insurance to an individual policy, you must include a deposit covering the monthly premium for a one-year term policy.
   
Coverage will take effect only after 31 days following the termination of your insurance.
   
If you die during the 31-day period following the termination of your insurance, provided you had applied to convert life insurance coverage, Industrial Alliance will pay an amount equal to the converted amount of insurance prior to the termination of your insurance.

Can I convert my dependents' coverage to an individual policy?
