On the earliest of the following dates:

upon retirement (you can continue to be insured for an amount of $10,000, provided you pay the required premiums); |

the date you choose to no longer participate in the Plan;
|
| |

the date you cease to participate in the Health Plan, unless you had opted out;
|
| |
|
| • |
upon your death; |
| |
|
| • |
the termination date of the Plan; and
|
| |
|
| • |
the date on which you are incarcerated after committing a criminal offence for which you were found guilty. |

| Additional Life Insurance Plan |
On the earliest of the following dates:

|
| • |
the first day of the month coinciding with or following your 65th birthday;
|
| |
|
| • |
upon your retirement;
|
| |
|
| • |
the date you choose to no longer participate in the Plan;
|
| |
|
| • |
upon your death; |
| |
|
| • |
the termination date of the Plan; and
|
| |
|
| • |
the date on which you are incarcerated after committing a criminal offence for which you were found guilty. |

When does my dependents' coverage end?
Your dependents' coverage under QPAT's Dependent Life Insurance Plan automatically ends at midnight on the earliest of the following dates:

| • |
your retirement date (you can continue to insure your spouse for $5,000, provided you pay the required premiums); |
| |
|
| • |
the termination date of the Plan; |
| |
|
| • |
the date on which your coverage ends; |
| |
|
| • |
the date on which your dependents cease to meet the definition of spouse or dependent children; and |
| |
|
| • |
the first day of the month after your employer has received written notice from you to the effect that you are no longer covering your dependents under QPAT's Dependent Life Insurance Plan. |

Can I convert my coverage to an individual policy?
Yes, in the following instances:
If your coverage ends because … |
You can convert your life insurance coverage, provided you are …

| Your employment or your group membership has been terminated (not because you simply opted out of the Plans) |
|
Your group coverage is cancelled or your group policy was modified
|
Under age 65, and you have been covered for at least five consecutive years
|
