How do I claim eligible drug expenses?

  1. You should have received a deferred-payment drug card from Industrial Alliance. Simply present this card to your pharmacist. Your card should show the number provided by ESI Canada.

  2. Pay the total cost of your prescription, including the portion that will be reimbursed by the Health Plan. The required information to process your claim will be electronically transmitted to Industrial Alliance.

  3. You have no claim form to complete. You will receive a reimbursement cheque from Industrial Alliance within two weeks.

    If the ESI system is not offered in your area, you have to fill out the Claim Request form and return it to:

Claims submitted with the form usually are processed within three weeks. You can phone Industrial Alliance's automated information system to determine your claim status. Simply call:

How do I claim other eligible expenses?

  1. Complete the Claim Request form. If you incurred dental expenses resulting from an accident, complete a Claim Request form (dental accident) instead.

  2. Return the form to:

Note: When your transportation is arranged by the Medical Assistance Service, you must present the original return ticket or the receipt. If neither is available, Industrial Alliance will withhold the price of the ticket from the amounts payable to you, if applicable.

What is the deadline for submitting health claims?

You have:

   
12 months from the date you incurred the eligible expense; or
   
if earlier, 90 days following the date your coverage ends.

What is the Plan's policy number?

The policy number is 97000 or 97001. Please refer to your wallet card for the applicable number.

Afterward, if a balance remains, submit a claim to your spouse's plan or the plan where you are insured as a retiree.

Your spouse incurs
Submit a claim to your spouse's plan first.

Afterward, if a balance remains and your spouse is also covered under QPAT's Health Plan, submit a claim to the Plan.
Your insured dependent children incur
Submit a claim to the plan of the parent whose birthday falls earlier in the year.

Afterward, if a balance remains, submit a claim to the other spouse's plan.

Who can help me if I have questions about a claim?

You can get information on your claims at any time, day or night, through Industrial Alliance's automated information system. With this system, you can also receive a confidential fax showing the last five claims paid.

The system is user-friendly and preserves the confidentiality of your personal information, since access is obtained through a personal identification number (PIN). If you are a first-time user, you must enter the month and date of your birthday (e.g., 05-27).
