When does my dependents' coverage end?


When does my coverage end?

Your coverage under QPAT's Dental Plan automatically ends at midnight on the earliest of the following dates:

   
•
the termination date of the Plan;
   
•
the date on which you cease to meet the eligibility requirements;
   
• the date on which you cease to participate in the Dental Plan;
   
• the date on which you are incarcerated after committing a criminal offence for which you were found guilty;
   
• your retirement date; and
   
• the date of your death.

When does my dependents' coverage end?

Your dependents' coverage under QPAT's Dental Plan automatically ends at midnight on the earliest of the following dates:

   
•
the termination date of the benefit or of the Plan;
   
•
the date on which your coverage ends;
   
• the date on which your dependents cease to meet the definition of spouse or dependent children; and
   
• the first day of the month after your employer has received written notice from you to the effect that you are no longer covering your dependents under QPAT's Dental Plan.
