What is the claims procedure if I also have coverage elsewhere?
If you injure your natural teeth and require dental care as a result, expenses will first be reimbursed by other plans that cover such expenses, such as QPAT's Health Plan. The portion of expenses that is not covered providing reimbursement of expenses incurred following injury to natural teeth is payable in accordance with the terms and limitations of QPAT's Dental Plan.
Who can help me if I have questions about a claim? You can get information on your claims at any time, day or night, through Industrial Alliance's automated information system. With this system, you can also receive a confidential fax showing the last five claims paid. The system is user-friendly and preserves the confidentiality of your personal information, since access is obtained through a personal identification number (PIN). If you are a first-time user, you must enter the month and date of your birthday (e.g., 05-27).
