From part-time to full-time status

You may opt out of the Long-term Disability Plan.
You may also choose to enroll your spouse and dependent children in only the drug portion of the Health Plan.

What you need to do
- Complete the Participation Request form, indicating the changes to your coverage.
- If you are a part-time teacher and you apply for long-term disability coverage at least 60 days after the date you became eligible, you must also provide proof of good health.
- Return all applicable documents to your school board.

New spouse

Health Plan
|
You must enroll your spouse in the Health Plan, unless he or she has coverage elsewhere. If you are a part-time teacher, you may choose to enroll your spouse either in the entire Health Plan or only the drug portion.

If you participate in the Dental Plan (for English Montreal School Board), you may enroll your spouse.

| Life Insurance Plans and Accident Insurance Plan |
You may choose to cover your spouse.
You may also wish to designate your spouse as your beneficiary and increase your life insurance coverage for yourself.

What you need to do
- Complete the Participation Request form, indicating the changes to your coverage.
- If you are applying for life insurance for your spouse or increased life insurance coverage for yourself, also provide proof of good health.
- If you are eligible for coverage under the Dental Plan and wish to enroll your spouse, also complete the Participation Request form for dental coverage.
- Return all applicable documents to your school board within 31 days from the date you have your new spouse.

Health Plan
|
You must enroll your dependent child in the Health Plan, unless he or she has coverage elsewhere. If you are a part-time teacher, you may choose to enroll your child either in the entire Health Plan or only the drug portion.

If you participate in the Dental Plan (for English Montreal School Board), you may enroll your child.

| Life Insurance Plans and Accident Insurance Plan |
You may choose to cover your child.
You may also wish to increase your life insurance coverage for yourself.

Disability

During the first six months of disability
|
You must continue to pay the required premiums under all Plans.

| After the first six months of disability |
You may request a waiver of premiums under all Plans, subject to the following conditions:

You are under age 65 when you become disabled. |
| |
|

You became disabled before your termination of employment and while insured under the Plans. |
| |
|
| • |
You have been disabled for at least six continuous months and provide satisfactory proof of disability, at your own expense, within nine months of becoming disabled. |

The amount of life insurance for which a waiver of premiums is granted will not be greater than that which was in force when you became disabled. This amount will be subject to change as if you were actively at work. |

|
| • |
You provide satisfactory proof of disability, at your own expense, as often as Industrial Alliance reasonably requires. |
The waiver of premiums terminates on the earliest of the date on which:

| • |
you cease to be disabled; |
| |
|
| • |
you fail to submit to an examination by the physician designated by Industrial Alliance; |
| |
|
| • |
you retire or reach your normal retirement age under your pension plan; |
| |
|
| • |
you reach age 65; |
| |

you fail to provide any required proof of disability; and |
| |
|

coverage for which premiums have been waived terminates. |
|
| During the first 104 weeks of disability |
Your school board may continue a percentage of your salary: 70% the first year and 66.6% the second year. |
| After 104 weeks of total disability |
Benefits become payable from the Long-term Disability Plan. |

Leave of absence
|
Leave with pay
|
Your coverage under the Health Plan, the Dental Plan, and the Life Insurance Plans continues. Payroll deductions also continue.
Your coverage under the Long-term Disability Plan also continues. However, if you become disabled while on leave, your disability will be considered to have begun on the day you would normally have returned to work.

| Leave without pay |
Your coverage under the Health Plan, the Dental Plan and the Life Insurance Plans is suspended, unless you continue to pay the required premiums.
You must choose to continue your coverage under the Plans for the entire period of leave, or not at all. If you do not continue coverage during your leave, your coverage will resume automatically when you return to work.
If you pay the required premiums to continue your coverage under the Long-term Disability Plan and you become disabled while on leave, your disability will be considered to have begun on the day you would normally have returned to work.

What you need to do
- If you are going on leave with pay, you don't need to do anything.
- If you are going on leave without pay, make arrangements with your school board to continue the payment of your required premiums.
|

Termination of employment
If you were dismissed and contest your dismissal with a grievance or arbitration under labour law, you may remain covered under the Plans in which you were participating until judgment is made, by paying the required premiums through your school board.

What you need to do
- Submit any outstanding health claims to Industrial Alliance.
- If you are part of the English Montreal School Board and participate in the Dental Plan, also submit any outstanding dental claims.
- Call Industrial Alliance to find out the cost of converting your life insurance coverage to an individual policy. If you wish to convert your coverage, you have 31 days after your termination of employment to submit a written request to Industrial Alliance. You must also include payment covering the monthly premium for a one-year term policy.

Divorce or separation

Your spouse's coverage, if any, ends.
To submit claims for expenses incurred by your spouse, you have:

if earlier, 90 days following the date coverage ends. |
You may want to change your category of coverage.
Also, if you had opted out because you were covered under your spouse's plan, you must enroll in QPAT's Health Plan. You must also enroll your dependent children unless they remain covered under your spouse's plan.

Your spouse's coverage (for English Montreal School Board), if any, ends.
To submit claims for expenses incurred by your spouse, you have:

- Submit your spouse's outstanding health claims to Industrial Alliance
- If you are part of the English Montreal School Board and your spouse participates in the Dental Plan, also submit your spouse's outstanding dental claims.
- Complete the Participation Request form, indicating any changes to your coverage and beneficiary designation. If you are eligible for coverage under the Dental Plan, also complete the Participation Request Form for dental coverage. Return the completed form to your school board along with satisfactory proof that you no longer have coverage elsewhere within 31 days of losing that coverage.

Retirement

Health Plan
|
Coverage under the Plan for active employees ends at retirement or age 65, whichever occurs first.
To submit claims under the Plan for active employees, you have:

12 months from the date you incurred the eligible expense; or |
| |
|
| • |
if earlier, 90 days following the date your coverage ends. |
At retirement, you become eligible for QPAT's Health Plan for retirees, without having to provide any proof of good health. The rates and conditions will be determined when you retire.
If you are age 65 or over and a Quebec resident, you automatically join the government's universal drug plan for drugs appearing on the government's formulary. If you prefer, you can get the same coverage as the government plan under QPAT's Health Plan for retirees, but at a higher cost. As a result, unless you inform QPAT otherwise, we will assume that you will remain with the government plan. Whichever plan you choose for drug coverage does not affect your coverage of other eligible expenses under QPAT's Health Plan for retirees.

- If you are part of the English Montreal School Board and participate in the Dental Plan, also submit any outstanding dental claims.
- To enroll in QPAT's Health Plan for retirees, send a written request to Industrial Alliance within 31 days of retirement.
- Call Industrial Alliance to find out the cost of converting your life insurance coverage to an individual policy. If you are under age 65 and wish to convert your coverage, you have 31 days after your retirement to submit a written request to Industrial Alliance. You must also include payment covering the monthly premium for a one-year term policy.

Death

Your beneficiary will receive any benefits payable under the Life Insurance Plans.

If the deceased was insured under the Life Insurance Plans, benefits will be payable to you.
You are entitled to change your coverage under the Health Plan and the Dental Plan.
You may wish to change your beneficiary under the Life Insurance Plans, subject to the provisions of the law.
Claims under the Health Plan and Dental Plan must be submitted within:

What needs to be done
- For life insurance benefits, have the Claim Request form completed. Return the form, and any supporting documents requested in the form, to the school board within 90 days immediately following the insured person's death. Proof of death must be submitted within five months immediately following the date benefits became payable (unless it is demonstrated that the documents were submitted as soon as possible).
- Submit any outstanding health claims to Industrial Alliance.
- If you are part of the English Montreal School Board and participate in the Dental Plan, also submit any outstanding dental claims.
- If you wish to change coverage as a result of the death of your spouse or a dependent child, complete the Participation Request form and, if you are eligible for coverage under the Dental Plan, the Participation Request form for dental coverage. Return all applicable documents to your school board within 31 days from the date of the dependent's death.
