When does my coverage end?

You coverage ends on the earliest of the following dates:

   
•
January 1 coinciding with or following your
75th birthday;
   
•
the date you choose to no longer participate
in the Plan;
   
•
the date you cease to participate in the Health Plan, unless you had opted out;
   
• upon your death;
   
•
the termination date of the benefit or of the Plan; and
   
• the date on which you are incarcerated after committing a criminal offence for which you were found guilty.

Note: If earlier, any additional optional coverage also ends on the first day of the month following your 65th birthday.

When does my spouse's coverage end?

Your spouse's coverage ends on the earliest of the following dates:

   
• January 1 coinciding with or following your 75th birthday;
   
•
the date your spouse no longer meets the Plan's definition of spouse;
   
• the first day of the month after QPAT receives written notice that you no longer wish to cover your spouse;
   
• upon your death;
   
• the termination date of the benefit or of the Plan; and
   
• the date on which you are incarcerated after committing a criminal offence for which you were found guilty.
