Note: To be eligible to join the Plan, you must have been covered under the Life Insurance Plan for active members of QPAT on the day before your retirement date. You must also be participating in QPAT's Health Plan, unless you have coverage under another health plan elsewhere.

Is participation mandatory?
No. It's your choice. If you wish to join, you may choose to cover:

only your spouse; or |

both you and you spouse. |

When can I join?
You must be a retired member of QPAT to be eligible for coverage. You have 60 days following your retirement date to submit a Participation Request form.

When can my spouse join?
Your spouse can join the Life Insurance Plan at any time once:

you join the Plan; or |
| |
|
| • |
he or she becomes eligible, according to the Plan's definition, if later. |
If your spouse applies later, proof of good health will be required.

Is proof of good health required?
Yes, proof of good health is required ONLY for your spouse, if you did NOT apply for his or her coverage when you joined the Plan.
Proof of good health must be provided at your own expense.

How do I join?
- Complete a Participation Request form.
- Be sure to designate a beneficiary.
- If applicable, submit proof of good health.
- Return the completed form—and any proof of good health—to QPAT, at the address appearing at the top of the form.

When does coverage begin?
Your coverage takes effect on your retirement date, provided you joined within 60 days following your retirement date.

the first day of the month following Industrial Alliance's acceptance of your spouse's proof of good health, if required; and

the date on which your spouse meets the Plan's definition of spouse. |
