When does my coverage end?
Your coverage under QPAT's Health Plan automatically ends at midnight on the earliest of the following dates:

the termination date of the Plan; |
| |
|
| • |
the date on which you cease to meet the eligibility requirements; |
| |
|
| • |
the date on which you cease to participate in the Health Plan; |
| |
|
| • |
the date on which you are incarcerated after committing a criminal offence for which you were found guilty; |
| |
|
| • |
your retirement date; and |
| |
|
| • |
the date of your death. |
When does my dependents' coverage end?
Your dependents' coverage under QPAT's Health Plan automatically ends at midnight on the earliest of the following dates:
| • |
the termination date of the benefit or of the Plan; |
| |
|
| • |
the date on which your coverage ends; |
| |
|
| • |
the date on which your dependents cease to meet the definition of spouse or dependent children; and |
| |
|
| • |
the first day of the month after your employer has received written notice from you to the effect that you are no longer covering your dependents under QPAT's Health Plan. |
