When does my coverage end?

On the earliest of the following dates:

   
• your 75th birthday;
   
• the date you choose to no longer participate in the Plan;
   
• upon your death; and
   
• the termination date of the Plan.

When does my dependents' coverage end?

Your dependents' coverage under QPAT's Accident Insurance Plan automatically ends at midnight on the earliest of the following dates:

   
•

the termination date of the Plan;
   
• the date on which your coverage ends;
   
• the date on which your dependents cease to meet the definition of spouse or dependent children; and
   
•

the first day of the month after QPAT has received written notice from you to the effect that you are no longer covering your dependents under QPAT's Accident Insurance Plan.
