How much does the Plan pay if I accidentally die?
You can buy from $25,000 to $350,000 in accident insurance for yourself, in units of $25,000.
If you are in an accident and die within 365 days as a result, the Plan will pay your beneficiary a lump sum equal to your full coverage amount.
In the event of accidental death, the plan may pay other benefits as well.

How much does the Plan pay if a family member accidentally dies?
The Plan will pay you a lump-sum benefit, provided:

How much does the Plan pay if I suffer an accidental injury?
If you are in an accident and, within 12 months, suffer an injury as a result, the Plan will pay a lump sum equal to a percentage of your full coverage amount ($25,000 to $350,000), depending on the covered loss.

If your full coverage amount is equal to $100,000 and you lose one arm or the use of one arm, the Plan will pay:

Any benefits payable for loss of use will be paid only if such loss:

How much does the Plan pay if a family member suffers an accidental injury?
The Plan will pay you a lump-sum benefit, provided:

Important: Except in the event of death, any approved benefit payable under this plan doubles for any dependent child, up to two times the amount payable in the event of accidental death, subject to a maximum of $75,000.

If your full coverage amount is equal to $100,000 and you lose one arm or the use of one arm, the Plan will pay:

In the event of accidental injury, the plan may pay other benefits as well.
What happens in the event of multiple injuries?
The Plan can only pay for one injury per person for any one accident. As a result, the benefit will be equal to the injury that pays the highest benefit.

Example
If you have one injury payable at 50% of your accident coverage, and another injury payable at 100%, the Plan will pay 100% of your accident coverage, not 150%.

What happens if more than one covered person is injured in the same accident?
The Plan will pay applicable benefits, subject to the Plan provisions and a maximum of $2.5 million for all losses resulting from any accident.
Also, if you and your covered spouse die within 90 days from injuries resulting from the same accident, the benefit payable for your spouse's death will be increased to the benefit payable for your death. The benefit payable for both deaths cannot exceed $700,000.
Does the Plan pay other benefits, such as rehabilitation?
Yes, the Plan pays the following benefits:

| Day-care benefit |
If you sustain an injury that results in death within 12 months of the date of accident, the Plan will pay the day-care benefit stated below for each dependent child under 13 years of age who:

is enrolled in a legally licensed day-care centre on the date of such loss; or |
| • |
enrolls in a legally licensed day-care centre within 12 months after you death. |
For each eligible child, the benefit is:

|
| • |
the reasonable and necessary expense actually incurred; |
| |
|
| • |
subject to 5% of your amount of insurance and a maximum of $5,000 a year, for up to four consecutive years. |
The benefit will be paid each year upon receipt of satisfactory proof that the child is enrolled in a legally licensed day-care centre. Payment will not, however, be made for expenses incurred before your death or for room, board or other ordinary living, travelling or clothing expenses.
If a dependent child satisfies the requirements indicated above, the day-care benefit will be payable to the surviving spouse, provided the spouse has custody of the dependent child. If there is no surviving spouse or the dependent child does not reside with the spouse, the benefit will then be paid to the child's legally appointed guardian.
"Dependent child" includes a child (or children of a multiple birth) who is born within nine months following your death.

| Education benefit |
If you sustain an injury that results in death within 12 months of the date of accident, the Plan will pay the education benefit stated below for each dependent child who is enrolled as a full-time student:

in a school for higher learning above the secondary school level, as defined in the province, territory or country of residence; or
|
| |
|
| • |
at the secondary school level but who enrolls as a full-time student in a school for higher learning within 12 months after the date your death.
|
For each eligible child, the benefit is:

the reasonable and necessary expense actually incurred; |
| |
|
| • |
subject to 5% of the insured person's amount of insurance and a maximum of $5,000 a year, for up to four consecutive years, provided the child remains a full-time student in a school for higher learning. |
The benefit will be paid each year upon receipt of satisfactory proof that the child is enrolled as a full-time student in a school for higher learning. Payment will not, however, be made for expenses incurred before your death or for room, board or other ordinary living, travelling or clothing expenses.
A dependent child who satisfies the above requirements will be deemed the beneficiary of the education benefit payable.
If, at the time of loss, you have no dependent children eligible for the education benefit, the Plan will pay an additional amount of $2,500 to the designated beneficiary.

| Family transportation benefit |
When, following a covered injury, a person under the regular care of a physician is confined as an inpatient in a hospital located at least 150 kilometres from the normal place of residence, the Plan will pay:

the reasonable and necessary expense actually incurred by any member of the immediate family for accommodation/lodging in the vicinity of the hospital; and |
| |
|
| • |
transportation of the family member, to and from the hospital, by the most direct route to and from the normal place of residence of the family member. |
Payment will not be made for board or other ordinary living, travelling or clothing expenses. If transportation occurs in a vehicle or device other than one operated under a license for the conveyance of passengers for hire, then reimbursement of transportation expenses will be limited to $0.20 per kilometre travelled.
The maximum amount payable is $10,000 for all such expenses.

| Home alteration and vehicle modification benefit |
If an injury sustained by an insured person does not cause loss of life, but results in a coverage loss that subsequently requires the insured person to use a wheelchair to be ambulatory, the Plan will pay the reasonable and necessary expenses actually incurred within three years following the accident for:

alterations to the insured person's principal residence; and
|
| |
|
| • |
modifications approved by the provincial vehicle licencing authorities to one motor vehicle used by the insured person;
|
| |
|
|
when such modifications are required for the purpose of making them wheelchair accessible.
|
|
This benefit is subject to a maximum of $10,000.
|

|
| Hospital indemnity expense |
A daily benefit of one-thirtieth of one percent of the insured person's amount of insurance, to a maximum monthly benefit of $1,000, is payable when the insured person is hospitalized and under the regular care of a physician, but only if such period of hospitalization:

is necessary for the treatment of an injury which results in a covered loss; and |
| |
|
| • |
begins while insurance is in force.
|
Such daily benefit will be paid from the first day of a necessary period of hospitalization as an inpatient, for which a full day's room and board is charged, but in no event for more than 12 months per accident.
A period of hospitalization which becomes necessary for the treatment of any injury other than for a covered loss will be covered in accordance with the above terms, and the daily benefit will be paid from the first day of hospitalization of at least a four-day period of hospitalization.
If a particular condition causes more than one period of hospitalization due to the same or related causes, then the maximum benefit (12 months in a hospital) will be reinstated, provided a period of 6 months has elapsed between periods of hospitalization.

| Repatriation benefit |
If an injury sustained by an insured person results in loss of life and an indemnity becomes payable, the Plan will pay the reasonable and necessary expenses actually incurred for the transportation of the body to the city of residence, including the preparation of the body for such transportation, subject to a maximum of $10,000.

| Seat-belt benefit |
If, due to a vehicular accident, injury results in a covered loss payable to an insured person, the insured person's amount of insurance will be increased by 10% if, at the time of the accident, the insured person was driving or riding in a vehicle and wearing a properly fastened seat belt.
The driver of the vehicle must hold a current and valid driver's license of a rating authorizing him to operate such vehicle and neither be intoxicated nor under the influence of drugs, unless such drugs are taken as prescribed by a physician, at the time of the accident. "Intoxicated" and "under the influence of drugs" are as defined by the local jurisdiction where the accident occurs.
Due proof of seat-belt use must be provided as part of the written proof of loss.

| Spousal retraining benefit |
If you die as a result of an accidental injury, the Plan will pay the reasonable and necessary expenses actually incurred within three years from the date of such accident by your spouse for formal occupational training to become specifically qualified for active employment in an occupation for which your spouse would not otherwise have sufficient qualifications, to a maximum of $10,000 for all such expenses. Payment will not be made for room, board or other ordinary living, travelling or clothing expenses.
In this case, your spouse will be the beneficiary of the spouse retraining benefit.

| Waiver of premiums |
If you become totally disabled and your application for a waiver of premiums is accepted and approved by the insurer in accordance with the Life Insurance Plan, the premiums payable under the Plan will be waived until the earliest of the date of your death, the date on which you are no longer disabled, your 65th birthday, and the date on which the Plan is terminated.

| Conversion privilege |
Upon termination of coverage, you may request, within 31 days following termination and without proof of insurability, that your accident insurance (but not the accident insurance for your spouse or dependent children) be converted into an individual policy for an amount equal to or less than the value of your policy on the date your coverage terminated. Premiums for the individual insurance are payable in advance on an annual basis.

Can I change coverage or stop participating?
Yes. You can change the amount of your coverage or stop participating altogether at any time.
To make a change:
- Complete the Participation Request form.
- Return the completed form to QPAT.

Are benefits taxable?
No.

When are benefits NOT payable?
Benefits are not payable when accidental death or injury results from:

suicide or attempted suicide while sane or insane; |
| |
|
| • |
intentionally self-inflicted injury; |
| |
|
| • |
flying as a pilot or crew member in any aircraft, or flying in an aircraft owned, operated or leased by or on behalf of QPAT; |
| |
|
| • |
declared or undeclared war or act of war; or |
| |
|
| • |
full-time active service in the armed forces of any country. |
